SUCCESS in Speech-Language Pathology:
Strengthening Underserved Communities through Enhanced Student Support

The College of Health Sciences has received a grant from HRSA to provide scholarships to full-time graduate students studying speech-language pathology who 1) have unmet financial need and 2) meet specified requirements demonstrating that they are from economic, environmental, or educational backgrounds that may place them at a disadvantage. The purpose of this scholarship program is to increase the number of licensed speech-language pathologists in medically underserved communities by removing financial barriers to successful program completion.

READ GENERAL INSTRUCTIONS BEFORE BEGINNING THIS APPLICATION PROCESS:

Carefully examine your email notification regarding the SDS scholarship program.

  • If the email states that you meet selected criteria for ‘disadvantaged,’ click on the boxes in STEP 2 that apply; no further action is needed for those criteria. If you determine that you meet additional criteria, click the additional box(es) and follow the instructions.
  • If no selection criteria for ‘disadvantaged’ are identified in your email, determine whether you meet any of the criteria listed in STEP 2, click the box(es), and follow the instructions.

Remember that you must meet criteria for both STEPS 2 and 3 to meet the minimum qualifications for the Scholarship. Follow instructions for both STEPS very carefully.

STEP 2: AREA(S) OF DISADVANTAGE

To qualify for this scholarship, you must meet at least one of the criteria below for ‘disadvantage’ based on the email that we send you.

  • If you received an email indicating that we have determined that you meet the criteria for ‘disadvantaged,’ check the items below that apply to you. No additional documentation is required.
  • If you received an email indicating that we have not been able to determine that you meet the criteria for ‘disadvantaged,’ check the items below that apply to you and provide any required documentation.
STEP 2 OPTIONS

Appalachian - To confirm whether you are from an Appalachian County, click here: www.arc.gov/counties. Click the box as appropriate.

First Generation - If neither of your parents graduated with a degree from a 4-year college or above, you meet the criteria. Click the box as appropriate.

High School with Low ACT or High Rate of Free/Reduced Lunch - If you were sent an email by the CSD program indicating that you attended such a high school, click the box.

Low Income Household - To determine whether you qualify in this category requires two steps and instructions vary depending on your particular age/dependent vs independent status.

First, determine your age category and dependent/independent status based on the descriptions below

Under 24- Regardless of your circumstances, you are considered a dependent. Therefore to document low-income status, you must submit a copy of the first 2 pages of your parental family 2023 family income tax return.

24-27 (Listed Dependent) - You are considered a dependent. Therefore to document low-income status, you must submit a copy of the first 2 pages of your parental family 2023 family income tax return.

24-27 (Unlisted Dependent) - There are two steps to complete.

  1. To verify that you are not a dependent, you must submit a copy of the first 2 pages of your parental family federal income tax return for the past 3 years (2021, 2022, 2023) showing that you were not claimed, and
  2. To document low-income status, you must submit a copy of the first 2 pages of your individual 2023 income tax return.

27+ - You are considered an independent. To document low-income status, you must submit a copy of the first 2 pages of your individual 2023 income tax return.

27+ - I am under 27 years of age and  am considered independent because of one of HRSA’s exception categories (e.g. marriage, children, veteran). Therefore to document low-income status, you must submit a copy of the first 2 pages of your individual 2023 income tax return.

Second, determine if the 2023 Federal income tax return is less than or equal to the highlighted income for family size in the table below

STEP 3: UNMET FINANCIAL NEED

If you meet any of the criteria in STEP 2, the next step is to establish the amount of unmet financial need. At a minimum, applicants must establish unmet financial need of at least 50% of the annual cost of tuition (combined fall & spring). This determination is made by the UK Office of Financial Aid based upon the current FAFSA and is established in accordance with HRSA and federal guidelines.  The UK Office of Financial Aid will inform Dr. Olson and Ms. Huffman of your calculated unmet need.

Submit the application and all supporting documents by August 5, 2023.
